Shanghai doctors develop software: mobile phone photographed children's stool upload can be screened for "biliary atresia"

Release date: 2016-04-25

Whether the color of the newborn stool is normal is closely related to whether or not the biliary atresia is present.

On April 24, 澎湃News () learned from the Pediatric Hospital affiliated to Fudan University in Shanghai that the biliary atresia diagnosis and treatment center expert designed a biliary atresia screening software called “Newborn stool color detector”. This is also the first biliary atresia screening software in China.

The parents download the software client through the mobile phone. The software has a standardized baby stool color card, which can identify the abnormal color of the stool color through the saturation value of the stool color uploaded by the parents. At the same time, the professional advice and guidance of the biliary atresia screening group of the diagnosis and treatment center is obtained. .

Stool color can reflect the possibility of biliary atresia

Biliary atresia is one of the most serious pathological jaundices and the most common cause of liver transplantation in children. In children with this disease, the biliary system is gradually occluded after birth, and bile can not be discharged into the intestines, so the baby will have severe jaundice, which will cause the liver to harden rapidly.

And how does biliary atresia relate to neonatal stool?

The color-receiving interface of "Newborn stool color detector" "lighter stool color is the most important early manifestation of biliary atresia. Suspicious cases of biliary atresia can be found by examining stool color." Experts from the biliary atresia diagnosis and treatment center of the Pediatric Hospital affiliated to Fudan University said For newborns with jaundice, the need to be vigilant is that the color of the stool is "lighter." Normal newborn stool is dark yellow, brown or green. When the bile excretion is blocked, the color of the stool becomes yellowish or even white. It should be emphasized that most of the biliary atresia of the child's stool is not "white", but pale yellow, so As long as the stool continues to "shallow", you should be alert to the possibility of biliary atresia.

It is reported that at present, the time of treatment and operation of children with biliary atresia in China is generally late, and only a small number of children undergo surgery before 60 days.

"This is because parents of newborns often have a misunderstanding of 'physiological jaundice', which delays the timing of pathological jaundice." Pediatric hospital experts pointed out that neonatal jaundice has pathological and physiological points.

"Physiologic jaundice" occurs 24 hours after birth, peaks 2-4 days after birth, and disappears within two weeks. Premature babies can last for 3 weeks. In addition, there is no deep urine color except for yellow skin. The stool color becomes lighter, and the yellowing of the skin is generally limited to the head, and the abdomen and the soles of the feet are generally not yellowed. The jaundice which is inconsistent with these conditions should be considered as pathological jaundice.

Experts also pointed out that neonatal pathological jaundice needs to be treated as early as possible, because the earlier the treatment of many diseases, the better the effect. Experts revealed that the treatment effect of biliary atresia is closely related to the age of surgery. Surgery is best performed 60 days ago. After more than 90 days of age, the effect is obviously worse.

Although the stool color card is an important tool for screening biliary atresia, there is no baby stool color card designed and developed independently in mainland China. In Taiwan and Japan, biliary atresia screening has been successfully carried out to improve the treatment of biliary atresia. effect.

Since January 2015, the Pediatric Hospital Biliary Atresia Center has cooperated with Shanghai First Maternal and Child Health Hospital, International Peace Maternal and Child Health Hospital, Fudan University Obstetrics and Gynecology Hospital and Changning District Maternal and Child Health Hospital to carry out the Shanghai Biliary Atresia Screening Project. Screening for biliary atresia in all live births born in four maternity hospitals. The screening method is to send a magnifying color card to the parents of the newborn, and to recover the color card and check the color of the stool when the newborn returns to the 42-day visit. The diagnosis and operation time of the biliary atresia in the screening process is significantly advanced.

To this end, pediatric hospital experts began to explore the production of their own stool color card. In the past six months, a standardized method of shooting was used to photograph the stools of preoperative and normal infants with biliary atresia, and representative photographs were taken to create a new baby stool color card. The color card will be applied to the Shanghai biliary atresia screening project from May this year.

In addition, parents often encounter difficulties in judging stool color, and there are cases of misjudgment, which leads to delays in the timing of biliary atresia. To this end, the hospital tried to design mobile phone software to objectively judge the color of the stool. The software can be used as an effective indicator for objectively judging stool color based on the saturation of stool color.
At present, the software has been synchronized on the IOS and Android platforms. Parents just download the software, click the “stool color” button on the page, upload the newborn stool to the software, and click the “Start Analysis” button. The software is It can automatically analyze whether the color of the stool is abnormal. Once an abnormality occurs, the parents can upload the shooting result to the expert group through software, and get timely and professional advice and guidance. This promotion and application is expected to improve the screening efficiency of the biliary atresia screening project in Shanghai.

Experts advise: If the newborn parent finds the possibility of "pathological jaundice" in the baby, it is strongly recommended to use the software to check the stool color and get in touch with the diagnosis and treatment center to diagnose or exclude biliary atresia as early as possible. Other newborn parents can also use the software to learn about physiological jaundice and pathological jaundice, and check for the presence of "pathological jaundice" in the self-examination function.
